    Laos Battles Highest Southeast Asia Adolescent Pregnancy Rates Due to Child Marriage

    Recent research has spotlighted Laos as the country with the highest rate of adolescent pregnancies in Southeast Asia, a troubling statistic attributed to a complex interplay of sociocultural factors, including the prevalence of child marriage, male dominance over reproductive choices, and insufficient sexual health education.

    The study, titled “Understanding diverse pathways to adolescent pregnancy in Lao People’s Democratic Republic,” provides an in-depth look into the experiences of Laotian girls who become pregnant before the age of 18. The findings suggest that in Laos, marriage is often seen as the only acceptable response to adolescent pregnancy outside of a union. Young girls view this as a viable alternative to education and once married, couples face societal pressures to prove their fertility.

    One of the study’s primary revelations is the degree to which male partners control reproductive decision-making in these young relationships, including opposition to condom use. “The male partner’s control over reproductive decision-making was also a key factor in young pregnancies, including the decision not to use condoms,” noted Dr. Marie Habito of the Burnet Institute, emphasizing the importance of understanding these personal experiences to aid policy development.

    The research indicates that girls’ first sexual encounters often occur under the influence of alcohol, which can lead to forced sex, or with promises from the male involved to address potential pregnancies through marriage. Girls’ lack of sexual health knowledge also plays a significant role; many reported not receiving information about reproductive health until after marriage or childbirth. One participant shared, “They all told me to ‘be careful … [and] don’t focus on boys.’ They said if we are staying out late and playing around, we will have a baby. But I don’t really understand what they mean.”

    The pattern of adolescent pregnancy being a cause or consequence of child marriage is not unique to Laos, as similar findings emerged from Indonesia, according to the study funded by UNFPA-UNICEF. “Similar to our findings from Indonesia, in Laos, adolescent pregnancy was either a cause or consequence of child marriage and early union,” Dr. Habito stated.

    In response to these challenges, the Lao Government has employed the Noi Framework since 2016, targeting the needs of adolescent girls, including avoiding early pregnancy, as part of its Sustainable Development Goals (SDGs) strategies. The UNFPA is also contributing by investing in comprehensive sexuality education (CSE) and sexual health services to help young people navigate these issues.

    From the study’s qualitative data collection across Vientiane and Luang Namtha, it’s clear that programs and policies must ensure adolescent girls have access to detailed sexual and reproductive health information in a non-judgmental environment. Moreover, these programs should encourage the use of contraceptives and empower girls to negotiate sexual activity and contraceptive use.
