The brains of first-time mothers are subject to dramatic changes in structure and volume during the late stages of pregnancy and the early postpartum period, a comprehensive study published in Nature Neuroscience has unveiled. These changes, documented through the largest longitudinal neuroimaging research of mothers to date, illuminate the profound neuroplasticity that occurs as women prepare for and adapt to motherhood.
Researchers have long suspected that the journey of pregnancy and transitioning to parenthood ushers in a period of significant neural transformation. Prior studies yielded conflicting findings, with some indicating a decrease in brain volume during pregnancy and others suggesting postpartum increases. This ambiguity prompted a more detailed exploration, aimed at creating a clearer depiction of the maternal brain’s metamorphosis.
Susana Carmona, the lead study author and head of the NeuroMaternal Lab, highlighted the importance of this research. “The transition to motherhood is a transformative event that affects almost half of the world’s population, yet it has been largely unexplored from a neuroscientific perspective. It is urgent to approach the topic,” Carmona emphasized. Her analysis, derived from MRI scans of 110 first-time mothers, showcased a global cortical volume and thickness decrease in these women during late pregnancy compared to their nulliparous counterparts.
This reduction was not confined to a specific area but was widespread, affecting midline regions such as the medial prefrontal gyrus, anterior and posterior cingulate, and lateral areas including the precentral and postcentral sulci, dorsolateral prefrontal cortex, and the temporoparietal junction.
Intriguingly, there was an observed increase in global cortical volume, thickness, and surface area from late pregnancy to early postpartum among mothers. This was particularly pronounced in regions associated with sensory processing and social interaction. The nature of childbirth was also found to influence these changes, with those undergoing scheduled cesarean sections showing larger postpartum increases in cortical metrics than mothers who experienced labor.
“Pregnancy, childbirth, and postpartum are characterized by very different hormonal, immunological, and environmental processes,” Carmona explained to PsyPost. “This study has enabled us to identify that these three stages also involve distinct neuroplasticity mechanisms, thus highlighting the necessity to evaluate how each of them uniquely shapes the maternal brain.”
Echoing these observations, a separate study led by neuroscientist Elseline Hoekzema also confirmed lasting gray matter reductions in first-time mothers. These brain alterations persisted for at least two years post-birth and were found to influence mothers’ ability to connect emotionally with their infants.
Despite these remarkable findings, limitations remain. The absence of pre-pregnancy brain data and endocrine and immune biomarkers in the study leaves unanswered questions regarding the complete scope of brain changes throughout motherhood. Nonetheless, Carmona and her team have set their sights on a future objective:”to track the complete trajectory of brain changes from preconception to several years postpartum.”
As science delves deeper into the mysteries of the maternal brain, these studies not only provide insights into the neural adaptations accompanying first-time motherhood but also underscore the possibility of using this knowledge to anticipate and address perinatal mental health challenges. The transformation from woman to mother, it seems, is not only an emotional and physical journey but a profoundly neurological one as well.
